What is an OPG dental X-ray?
OPG stands for orthopantomogram. It is a two-dimensional panoramic image of the upper and lower jaws. Instead of placing a small film or sensor inside the mouth, the machine moves around the head while you remain still. This produces one wide image that helps the dentist assess areas that are difficult to see during a routine examination.
An OPG does not replace every other type of dental X-ray. Small bitewing or periapical images can show individual teeth in greater detail, while a CBCT scan creates a three-dimensional view for selected complex cases. Your dentist chooses the image that answers the clinical question with the lowest appropriate exposure.
When might a dentist recommend a panoramic X-ray?
A panoramic dental X-ray may be useful when planning or assessing:
- wisdom teeth and teeth that have not fully erupted;
- multiple missing teeth or dental implant options;
- jawbone levels and broader signs of gum disease;
- large infections, cyst-like areas or other changes in the jaws;
- orthodontic or Invisalign planning;
- complex restorative treatment involving several teeth;
- jaw joints, facial trauma or unexplained symptoms where a wider view is needed.
The decision depends on your symptoms, dental history, examination and any recent images already available. X-rays are not taken routinely without a clinical reason.
What can an OPG show?
An OPG can show the position and development of teeth, the general condition of the jawbone, the roots of many teeth and the relationship between teeth and nearby anatomical structures. It can also help identify areas that need a closer examination or a more detailed image.
Because it is a broad overview, an OPG may not show early decay between individual teeth as clearly as bitewing X-rays. If a specific tooth is painful or may need root canal treatment, the dentist may also request a focused periapical X-ray.
What happens during the appointment?
The radiographer or dental team member will ask you to remove glasses, earrings, necklaces, removable dentures and other metal objects around the head and neck. You will stand or sit at the machine, rest your chin in position and bite gently on a clean guide. Supports help keep your head still.
The scanner then rotates around your head. The exposure itself normally takes only several seconds and is painless. You will not feel the X-rays. Remaining still is important because movement can blur the image and may mean it needs to be repeated.
Is an OPG X-ray safe?
Dental X-rays use ionising radiation, so every exposure should be justified and kept as low as reasonably practicable. An OPG uses a relatively low dose compared with many medical imaging procedures, but it is still taken only when the expected diagnostic benefit outweighs the small radiation risk.
Tell the dental team if you are pregnant or think you may be pregnant. This does not automatically mean that an essential X-ray cannot be taken, but it helps the clinician make an informed decision about timing and protection.
OPG vs CBCT: what is the difference?
An OPG provides a flat panoramic overview. CBCT produces a three-dimensional image and can offer more detail for selected implant, surgical, endodontic or impacted-tooth cases. CBCT generally involves a higher radiation dose than an OPG, so it should be used only when the extra three-dimensional information is necessary for diagnosis or treatment planning.
